Research supports that a significant amount of information is lost when patients are transferred from one clinical area to another.  This results in increased risk to the patient, adverse patient events, longer length of stay, distrust between patient/family and health care workers and between health care workers along with other complications.  Following a few adverse patient events where data transfer was identified as a potential contributor, our Cardiac Program undertook a clinical process change to formalize the patient transfer process within our departments with the goals of reducing data loss and improve the patient and nurse experience when transferring patients. Following a review of the research and suggested document templates, we developed our own document and standard work based on identified points of information loss and critical information.  We trialed and evaluated our process making revision based on feedback from bedside staff, patients and other members of the health care team.  This led to adopting a formalized patient transfer process that includes both a written document and in-person face to face reporting.  This clinical process change has resulted in improved communication between the departments, clear expectations of the process, expressed satisfaction from nursing and patients/families, improved reports of trust between departments and between nursing and patients/families along with no further reported incidence of data loss. This presentation will provide a review of the research, the development and revision process as well as share lessons learned and next steps.
