Central obesity is an independent coronary heart disease (CHD) risk marker and patients with central obesity who undergo cardiac rehabilitation (CR) have to manage two comorbid conditions and might need more individualized treatment. The study aimed to investigate the effects of 12-week eHealth CR on the health outcomes of patients with CHD. A two-arm randomized controlled trial that allocated 78 CHD patients to receive either the 12-week eHealth CR (intervention group) or usual care (control group) was conducted. Underpinned by social cognitive theory, eHealth CR started with a face-to-face assessment and orientation session to identify individual risks for goal-setting and familiarize with the e-platform/device. Then, patients were invited to use the CR website for knowledge and skills learning, health data uploading, the pedometer for step tracking, and interacting with peers and professionals via WeChat. Data were collected at baseline, six-week (T1), and 12-week (T2). Participants in the intervention group improved their daily steps at six weeks (β=2713.48, p=.03) but not at 12 weeks. Improvement in weekly sitting minutes, total (vigorous, moderate, and walking) exercise, health-promoting lifestyle profile, smoking cessation, self-efficacy, body mass index and waist circumferences at T2 were identified.  Central obesity should be identified in eHealth CR intervention. Individual assessment, patient-centered collaborative action planning, and ongoing monitoring and feedback support should be incorporated as essential ingredients.
