Heart failure (HF) is a growing epidemic with an estimated annual $2.8 billion cost to Canadian taxpayers. Patients with HF follow a clinical course of frequent emergency department visits and hospital readmissions, driving costs. Prompt follow up with HF patients post-hospital discharge has been associated with lower rates of readmission and death. In May of 2021, the HF program in Calgary, Alberta piloted the Heart Failure Continuity Clinic (HFCC), a nurse practitioner (NP) run, cardiologist supported initiative. The HFCC is aimed at early assessment and management of HF patients, along with prompt optimization of their guideline directed medical therapy (GDMT) using a novel algorithm. Due to the great need and its successes, HFCC has been adopted as a permanent clinic. There are several recommended therapies for heart failure with a reduced ejection fraction (HFrEF). However, the best way to initiate and titrate these therapies to target doses is unknown. As a result, many patients are placed on suboptimal dose/combinations of HF medications, which contributes to adverse outcomes. This workshop will highlight a novel approach to optimizing GDMT for HFrEF, which is utilized in the HFCC. Real case examples will be included to showcase implementation of the algorithm and how this can be adopted safely in daily HF management. Consistent gaps exist post-hospital discharge, and we will also highlight how the HFCC has helped to bridge these gaps to avoid medication errors and hospital readmissions. Future goals and next steps for the HFCC, a NP-led HF initiative, will also be shared.
