Concurrent Session: Stop the silos! How nurse practitioners and pharmacists are breaking down barriers in community cardiovascular care

Nurse practitioners and pharmacists are poised to create and sustain collaborative care models to enhance patient care from many streams. Due to the burden of heart disease in Canada, providing expert patient care with implementation of, and adherence to, clinical practice guidelines is a top priority. A dedicated pharmacy program in Calgary, Alberta, called the Calgary COOP SPECIALTY HUB, has been identified as a key partner for collaborative care. A group of nurse practitioners in community cardiology, cardiovascular rehab and acute care have established a clear referral pathway and partnership with the Calgary COOP HUB pharmacy team to improve patient care for conditions including heart failure, lipid management, hypertension, type 2 diabetes and tobacco reduction. Collaborative care encompasses communication, joint decision making and respects the unique ability of each profession, while prioritizing each patient’s wellness and illness needs. This workshop will showcase the collaborative care between cardiac nurse practitioners and community pharmacists. Participants will be provided with concrete examples of how each profession, working to its full scope of practice, can provide safe and synergistic care to implement evidence-based medicine and improve patient outcomes. Representatives from each profession will highlight real case examples to enhance practical application. At the end of this workshop, participants will be able to identify new possibilities for referrals and partnership in their day-to-day clinical practice to optimize patient care.
