Early recovery after TAVI (ERT) clinical pathway: A pilot initiative

Transcatheter Aortic Valve Implantation (TAVI) has evolved to a highly predictable and relatively low-risk procedure. Increasing volumes of TAVI referrals coupled with the limited resources in staffing and catheterization room availability has posed challenges to timely access to care. With limited availability of anaesthesia resources to support TAVI procedures in the Vancouver program, we strategized to streamline TAVI care whist maintaining the safest care for patients. Since catheterization lab nurses are critically trained and well versed in administering sedation for other complex procedures, there role could be further extended to their full scope of practice within the TAVI procedure. We developed the Early Recovery after TAVI (ERT) clinical pathway initiative to evaluate the feasibility and safety of TAVI with physician led, nurse supported sedation with anaesthesiologist on-call.
This presentation will describe the ERT pathway, development of the criteria for safe patient selection and the pre-, peri-, and post-procedural nursing care protocols. Utilizing quality improvement methodology of PDSA (Plan-Do-Study-Act) cycles, we evaluated changes iteratively with multiple stakeholders including all participating patients.
