A Tale of Two QI Projects

New knowledge is being created constantly, however, it must be mobilized into practice to positively impact patients. Mobilizing knowledge into practice is a key component of the nursing role in healthcare and is commonly done through quality improvement (QI) projects. Although common in healthcare, QI projects are rarely published or shared at conferences, leading to reduced understanding of the process and, thus, reduced capacity among nurses. The objective of this presentation is to describe one QI framework, the Model for Improvement framework, and provide two case exemplars of QI using this framework. The case exemplars will focus on optimizing the patient journey related to transcatheter aortic valve implantation and the development of a regional pathway for optimal care of patients diagnosed with a ST-elevation myocardial infarction. We review the Model for Improvement framework and then discuss the work undertaken prior to the commencement of the case exemplars, including the forming of teams and the creation of project charters. We will share how aims were set for each case exemplar and how changes were measured and determined. We describe the plan, do, study, adjust cycle of each case exemplar. Finally, we will share key insights and learnings from the two case exemplars to support nurses undertaking the QI process. The implications for practice for this presentation are increased knowledge of and capacity for QI in the Canadian cardiovascular nursing and allied healthcare provider community. Together, this will improve care and patient outcomes for Canadians.
