An oral workshop presented by an advanced heart failure nurse practitioner with a subspecialty in palliative care. This case-based presentation and communication skill practice workshop will help the cardiovascular nurse navigate difficult goals-of-care (GOC) conversations from a value-based decision-making approach.

GOC conversations help patients with serious illness clarify what they value most and what they hope to achieve with their medical care. With growing options for cardiovascular disease treatment and interventions, GOC conversations are increasingly important to assist the nurse to advocate for patient preferences within the healthcare team. Despite their centrality in the team, nurses receive little formal education in developing strong communication skills. This complex skill remains underdeveloped in nursing, often leading to feelings of discomfort and decreases nurse initiative in facilitating these essential conversations.

The purpose of this workshop is to increase the nurses’ confidence in discussing death and dying and eliciting GOC from patients and families. The presenter will utilize case examples to facilitate a session on the barriers to hosting these conversations, strategies to address awkward moments, and introduce value-based decision making. Value-based decision making helps the nurse to elicit a patient’s core values and align them with difficult decisions required during advanced care planning and treatment decision making. By better understanding patient and family core values, the nurse can also help navigate decisions when options and values are in conflict. Through the incorporation of real-world examples, the presenter will provide an interactive learning environment with role play and independent reflection.
