Heart failure (HF) impacts many individuals and their families every year. In Canada, 750,000 people are living with HF and 100,000 are diagnosed each year (Heart & Stroke Canada, 2022). Education is key to engaging patients and families as active participants in their care and improving quality of life.

A committee of seven cardiac nurses was initiated to improve patient outcomes with the HF population on two cardiac units. In May 2019 the leadership team in cardiac sciences was approached by one of the lead HF physicians to develop educational videos for HF patients and their families. The intent of the video series was to highlight education about heart failure and share stories of patient’s lived experience and journeys of living with heart failure. The vision was for the video series to be accessible to all heart failure patients and families throughout the Calgary zone.

The nursing lead collaborative interviewed multidisciplinary team members who contributed to both in-patient and out-patient heart failure programs. Four education videos were created: What is Heart Failure; Heart Failure in the hospital; Transition to Home and Living with Heart Failure.

In February 2022, the HF video series was released with a QR code allowing readily accessible videos for both health care workers providing patient education and ongoing access for patients and families. A synopsis of the videos will be shared in this presentation.

The Calgary zone is benefitting from an easily accessible patient education video series that has received over 1200 views since February.
