Palliative Care for Canadian with Heart Failure

This talk will explore the Who, What, Were, When and Why of palliative care & heart failure. An overview of related Canadian Cardiovascular Society guidelines will be provided in addition to a description of the heart failure and palliative care relationship. To tie it all together, a personal patient/family experience will be shared, and conclude with ample time for questions, brainstorming, and sharing.
