A Rare Find: Takayasu Arteritis in a Young Female

Takayasu Arteritis is a rare, inflammatory vasculitis that affects the large vessels often with systemic manifestations.  It is most commonly found in women of childbearing age, this presentation will discuss takayasu arteritis as it presented in a 17 year old female.  This young lady presented with an 18 month history of low energy, fatigue, nausea, vomiting, and weight loss of 20 kgs.  During a routine checkup her family physician detected a new murmur and an echocardiogram revealed a massively dilated ascending aorta with sever aortic regurgitation.  Cardiovascular magnetic resonance imaging revealed involvement of the carotid arteries and intracerebral vessels. She was admitted to hospital at an acute inflammatory stage, with cardiology, rheumatology and cardiovascular surgery following her care. Medical treatment was initiated with IV steroids and immunosuppressive therapy.  After 6 weeks of treatment, a positive emission tomography (PET) scan revealed a lesser degree of inflammation and she was deemed ready for aortic surgery. She underwent a successful mechanical aortic valve, root reconstruction, coronary re-implantation, ascending and hemi-arch replacement. Her post-operative course was complicated by a high grade atrioventricular block, pericarditis and infection.

This presentation will provide an overview of the six different types of takayasu arteritis with a focus on type 2A, as this was the diagnosis of this young female. An overview of this interesting case will be discussed including her initial presentation, diagnosing characteristics, medical and surgical implications, post-operative challenges and long-term management.
